Members must fill out a Medical Assessment and History Questionnaire before the consult, and may request prescription medication.
- Tele-Consult By Appointment - For an in-depth, comprehensive consultation with a physician at a specific and convenient time, members can order a Tele-Consult By Appointment by calling 1-800-DOC-CONSULT (1-800-362-2667), or by logging into their member health portal at www.consultadr.com .
